At ReFocus Eye Health, we use the Ocular Surface Disease Index (OSDI) because it’s a validated, widely adopted tool for assessing dry eye symptoms and the impact on daily life. It helps us quantify how you feel today and track meaningful change over time.
- Clinically validated: The OSDI reliably separates normal, mild, moderate, and severe symptom levels and shows good test–retest consistency when symptoms haven’t changed.
- Aligned with guidelines: Major professional groups include patient-reported outcomes like OSDI in dry eye evaluation and research protocols.
- Actionable for care: Trends in your OSDI score help your doctor tailor treatments (e.g., tear film optimization, meibomian gland therapy, anti-inflammatory care) and confirm what’s working.
Your score is one piece of the full picture. We combine it with exam findings to design a plan that protects comfort and vision.

- Questions & values: Each non-N/A answer is scored 0–4 (0 = none of the time; 4 = all of the time).
- Formula: (Sum of answered items × 25) ÷ number of answered items.
- Bands for interpretation: 0–12 Normal, 13–22 Mild, 23–32 Moderate, 33–100 Severe.
- Why N/A matters: N/A responses are excluded from the denominator so your score reflects only relevant situations.
